- A physical or electronic signature of the copyright owner or a person authorized to act on their behalf.

- A statement that the information in the notice is accurate and, under penalty of perjury, that you are the owner or authorized to act on behalf of the owner.
Counter-notification
If you believe material was removed by mistake or misidentification, you may send a counter-notice to the same address including: your physical or electronic signature; identification of the material and the URL where it appeared before removal; a statement under penalty of perjury that you have a good-faith belief the material was removed by mistake or misidentification; and your name, address, telephone number and consent to the jurisdiction of the federal district court for your address (or, if outside the United States, any judicial district in which the service provider may be found), and that you will accept service of process from the person who provided the original notice. The material may be restored in 10 to 14 business days unless the claimant notifies us that a court action has been filed.
Repeat infringers
Sources or contributors who repeatedly infringe copyrights will have their material removed and will not be published again.
